Dietetic Support Agreement
The purpose of my dietetic consultation is to learn more about nutrition and how to live a balanced lifestyle. I understand that the guidance provided is intended to support my health and improve my relationship with food.
I agree to commit to working with Toni Rudd for a minimum of 3 months to see positive change and receive the necessary support (unless otherwise agreed upon during our free call). I understand that this commitment can be adjusted based on individual circumstances.
I acknowledge that Toni Rudd will take every care to share advice tailored to my lifestyle and based on the information provided during the initial consultation and any follow-up assessments. However, I accept that Toni assumes no responsibility or liability for any harm resulting from the application of any information or advice provided.
I understand that this is Toni's small business, and I will communicate honestly, respectfully, and promptly. I recognize the importance of maintaining open communication to ensure the success of our work together.
Appointment Scheduling & Sessions
Clinic Capacity: Toni allows a maximum of 4 clients per day to ensure the quality of service.
Session Duration:
Initial Assessment: 60 minutes
Follow-up Review Sessions: 45 minutes
Availability: Appointments are available on Tuesday, Wednesday, and Thursday. Fridays are available with flexible timing, and no appointments will be scheduled on Mondays.
Booking Policy
First Come, First Serve: All appointments are first come, first serve. Please book your preferred time as soon as possible to ensure availability.
Recurring Sessions: If you would like to schedule all of your purchased sessions on the same day and time for weekly or bi-weekly support, we can arrange this in advance to ensure consistency and convenience.
Session Preparation
Preparation for Sessions: If it has been 2 weeks or more since our last session, Toni requests that you complete a session prep form to help with time management and preparation. You will receive this 48 hours before your appointment to ensure time to complete.
Weekly Clients: The session prep form is not required if we meet weekly.
Post-Session Email Summary
After each session, Toni will send you an email summarising the goals we discussed and any tasks or homework assigned to work on before our next session. Toni will aim to send this within 24 hours of our session.
Technology and Online Consultations
I understand that there are potential risks associated with technology, including interruptions, unauthorised access, and technical difficulties.
I understand that Toni Rudd or I can discontinue the online consultation or switch to a voice call if it is felt that the video conferencing connection is inadequate for the session.
WhatsApp Messaging Policy
Response Time:
Toni will respond to messages within 24 hours, Tuesday to Thursday. Messages sent outside this time will be replied to on the next available practice day.Boundaries:
This is not an emergency service. If you're feeling overwhelmed or need urgent support, please contact a medical professional.
Please limit messages between sessions to one concern or issue per week.
Keep voice notes under 2 minutes and focus on one topic at a time so I can provide a full response.
Preferences for Communication: Let me know if you prefer voice notes or text messaging for our WhatsApp conversations so that I can tailor the experience to your comfort.
Cancellation and Late Arrivals
Cancellation Policy:
You can cancel or reschedule your appointment up to 24 hours before the session.
If you cancel within 24 hours of the session or fail to show up, you will lose that appointment without the opportunity to reschedule.
Late Arrivals:
Toni will allow a 15-minute grace period for late arrivals. After 5 minutes, Toni will send you an email/WhatsApp message to check if you’re running late. After 15 minutes, the appointment will be considered a no-show, or the session will be shortened to 30 minutes.
Time Off and Holidays
Toni will provide 1 month's notice via email for any upcoming holidays during which she will not be taking appointments.
If Toni is unable to attend the session due to illness, Toni will provide as much notice as possible and offer rescheduling options as soon as possible.
Regular Review Sessions
Every 3 months, we will schedule a review session where we can:
Reflect on your progress and goals.
Discuss any adjustments to your plan.
Gather your feedback on the service and systems used so I can identify areas for improvement.
Use of Personal Data
Your personal details will be handled by Toni Rudd in accordance with General Data Protection Regulations (GDPR). For further information on how your personal data is collected, used, and protected, please refer to the Privacy Policy and Terms & Conditions available on our website.
Refund Policy
All payments for services are non-refundable. If you choose to discontinue services before the agreed-upon period, any unused sessions will not be refunded. However, I am committed to working with you to address any concerns and ensure you receive the support you need.
Claims
In the event that any claims arise during the course of our work together, they will be governed by and subject to the laws of the United Kingdom Jurisdiction.
